The Donor Form is a legal requirement that allows us to process your donation and provide you with the 80G certificate. Without the Donor Form, we will not be able to proceed with the 80G certification process.

To ensure you receive the benefits of your donation, please download the Donor Form from the below link. Kindly complete the form with accurate information and send it along with the payment details.

To proceed with your donation, please prepare a letter as per the attached format and send it along with the transaction ID to donations@induscancer.com or contact us at +91 7995 553 717. You may download the donation form, duly fill it, and submit it with your payment for all NEFT transactions.

Please submit the Donor Form within one week of your donation to avoid any delays in obtaining your 80G certificate.
